SCHOLARSHIP APPLICATION
PLUS PT.DJARUM 2011

Assalamu'alaikum Wr. Wb.

In 2011-2012, PT. Djarum provides scholarship for S-1 regular students Rp. 600.000,-/month/student for a year with the following requirements:

 

1. Students of cohort 2009. Please attach your photocopy of Student ID card.
2. Minimal GPA is 3.00. Please include your academic transcript (signed by Chief of Department/Secretary of Department/Vice Dean III) and copy of the current KHS.
3. Attaching note on parent's salary from the institution they work in or from village government.
4. Attaching a letter stating that the students are not getting scholarship from another source. The letter is signed by Vice Dean III (form is available at the faculty office).
5. Students who are active in students activity must attach a letter signed by Vice Dean III or Chief of Interest and Talent Division.
6. Unmarried. Max 21 years old.
7. Attaching 2 (two) 4X6 color photos (wearing alma mater jacket, close up, front view)
8. Attaching copy of certificate (seminar, etc.) (if any).
9. Filling form of Djarum Scholarship application (form is available at Student Affairs Office or at http://kemahasiswaan.umm.ac.id)
10. DEADLINE August 11, 2011 at the faculty
